When should you make a report?
Submit an Incident Report if you are worried about the health and safety of an individual - or have witnessed a student struggle academically or personally - and are unsure how to assist.
Is someone you know:
- demonstrating disruptive and/or disturbing behavior?
- showing dramatic changes in appearance, behavior, and/or weight?
- having problems at home, with classes and/or work?
- making disturbing comments in conversation, email, letters, social media postings and/or papers?
- sad, anxious and/or experiencing dramatic mood shifts?
- abusing alcohol and/or drugs?
- isolating himself/herself socially?
- acting paranoid and/or suspicious?
- frequently angry and/or easily frustrated?
- struggling with health problems?
These behaviors, especially when more than one is present, may be signs of distress. Your confidential report to Coastline's Care Team can make a difference.

Our focus is care and concern for students, faculty, and staff members who may be in distress. We coordinate resources and implement a centralized response, with the goals of providing assistance to the individual and keeping our community healthy and safe.
What should you include in your report?
Providing as much information as possible is essential!
- Individual's name and ID number (if known).
- Brief factual description of the incident and/or behavior.
- Direct quotes whenever possible.
- Where and when the incident and/or behavior occurred.
- Names and contact information of witnesses.
- Your name, position, and complete contact information.
- Include all emails and/or other information you have.
- Always save voice recordings, text messages, and emails on the device that received them.
What happens after you make a report?
Coastline's CARE Team seeks to connect students to the proper department(s) that will provide the best resource(s). Care reports are reviewed during normal business hours by people who are trained to assess and act. We conduct an initial assessment on every report. In many cases, the outcome of the initial assessment is to offer support and resources to either the individual who reports the concern, the person of concern, or both.
We will collaborate with the person making the report and with other campus and community resources, if appropriate. In some cases, a more in-depth analysis may occur before the CARE Team can determine next steps of intervention. The reporting party will be kept informed, as appropriate, of the situation while mindful of the privacy of all parties involved.
If there is a concern for safety, the CARE Team can help those who report to contact the appropriate on- and off-campus support and resources. While maintaining privacy is the CARE Team's goal, please be aware that reports and other communication may be subject to review as outlined in federal and state laws. The campus does not tolerate retaliation.
If there is a potential threat, the CARE Team provides guidance and recommendations to the appropriate individuals to mitigate or manage the threat. The CARE Team retains all reported information to assist in identifying potential behavioral patterns.
